PURUSANT TO THE PERSONAL DATA PROTECTION LAW NO. 6698 CONCERNING THE PROTECTION OF PERSONAL DATA CUSTOMER EXPLICIT CONSENT DECLARATION FORM  

In accordance with the Personal Data Protection Law No. 6698 ("Law" or "PDPL") and its related legal sub-regulations, within the scope of the Disclosure Text presented to my knowledge by Blutrade Information Technologies Joint Stock Company ("Blutrade" or "Company"); for the purposes of receiving services for the performance of the contract, access requests (data, location, system), notifications for fulfilling legal obligations, internal audit, central control, investigation and inquiry activities, R&D activities, and execution of processes related to purchase requests;

The personal data listed above, such as the data subject's identification document, the expiration date of the identification document, first name, last name, date of birth, country of birth, city of birth, gender, marital status, nationality, Turkish ID card information (TCKN, serial no, wallet no, father's name, mother's name, place of birth, province, district, neighborhood, volume no, family row no, row no, household no, page no, registration no, place of issuance, reason for issuance, date of issuance, previous last name), copy of identity card, signature, address information, e-mail address, communication address, telephone number, call center records, account opening, transaction and amount information, request information, IP address information, website/mobile application entry and exit information, password and password information, financial performance information, money transfers made to the account, IBAN information, account history information, cookie records, information obtained through campaign work, photographic image, address document, front and back photos of the identification document, customer selfie, and a moving face video for identity verification

I accept, declare, and undertake that, in accordance with Article 10 of the Law, I have been informed about the recording, processing, storage, and transfer to domestic and foreign business partners of my personal data and sensitive personal data listed above, as well as the deletion, destruction or anonymization of my personal data in compliance with the relevant Law and its related legal sub-regulations upon the expiry of the legal storage period, that I have read and understood the Privacy Policy, Personal Data Protection and Processing Policy, and the Visitor Disclosure Text published on the official website of the Company, and that I explicitly consent to the collection, recording, processing, storage, and transfer of my personal and sensitive personal data by the Company as stated in this "Explicit Consent Declaration" without being under any influence or pressure.
